Description

Elmwood Residences Inc. is a community-based, not-for-profit organization serving individuals experiencing intellectual disabilities.

Position Overview
    Responsible for delivering on-site management and supervision of Elmwood (ERI) community homes including overseeing all aspects of resident and family support utilizing a Resident First Approach.

    Provide ongoing coaching, support and mentorship to Support Workers through a Resident First Approach.

    Provide orientation and training to Elmwood Staff Members and ensure that mandatory training is completed.

    In collaboration with Elmwood team members, interview, hire, supervise and discipline Support Workers working in Community Homes.

    Facilitate Person-Centered Planning (PCP) sessions and support the ongoing implementation of PCP goals in partnership with residents, families, Elmwood staff and community partners.

    Ensure resident care plans are reviewed, updated and implemented on ongoing basis through a strength-based approach.

    Submit and oversee incident reports for residents in homes focused on ensuring safety and wellbeing of residents.

    Monitor policy and procedural compliance in line with the Contract for Services of Community Living Service Delivery (CLSD) and Elmwood Residences Policies and Procedures.

    Monitor and oversee financial aspects in community homes including petty and resident discretionary funds.

    Develop and maintain positive working relationships with residents, families, staff and community partners.

    Other duties as assigned.

Qualifications
    Degree/Diploma in related discipline considered an asset.

    Experience working with individuals experiencing disabilities.

    CPIC with Vulnerable Sector Clearance.

    Class 5 Drivers License.
