Job Description: THE ASSESSOR / COORDINATOR - DEGREE IS ACCOUNTABLE FOR THE ONGOING DETERMINATION FOR THE CARE NEEDS OF THE HOME CARE CLIENTS OF THE PAPHR, THE COLLABORATIVE COORDINATION OF SERVICE AND THE CASE MANAGEMENT PROCESS.  THE DELIVERY OF CARE IS CONSISTENT WITH THE POLICIES, VISION, MISSION AND HEALTH OUTCOME GOALS OF THE REGION. THIS POSITION REPORTS DIRECTLY TO THE DESIGNATED CTM (COMMUNITY TEAM MANAGER).
Required Qualifications

Valid Class 5 driver's license
Bachelor Degree/Diploma in Nursing or Bachelor Degree/Diploma in Psychiatric Nursing or Bachelor Degree in Social Work
Registered or eligible for registration with the appropriate licensing body
Experience
EXPERIENCE IN ASSESSMENT, CASE MANAGEMENT AND/OR CRISIS INTERVENTION WITH INDIVIDUALS AND FAMILIES.  HOSPITAL AND/OR MEDICAL EXPERIENCE AN ASSET.
Knowledge, Skills and Abilities

Ability to work independently and as a member of a multi-disciplinary team
Ability to communicate with and maintain an effective working relationship with clients from diverse social, economic and cultural backgrounds
Ability to competently use computers and software applications as required by the position
Advanced leadership skills
Assessment skills
Case management skills
Access to a reliable vehicle
