Roles & Responsibilities:
Under the direction of the Health Director of Health, the community health nurse will:
-Meet the Community Health Nursing standard which will guide the Scope of Practice
-Prevention and Health Promotion
-Build community capacity by Education and promoting health and wellness, through engagement and consultation
-Engage in communicable disease follow-up and management
-Collect Data and reporting information as requested
-Collaborate with the EHO
-Make referrals to appropriate health care providers to ensure clients have access to appropriate health care
-Integrate prevention and health protection activities
-Provide breastfeeding support/education support
-Utilize screening tools in Child Health Clinics
-Assess maternal mental health prenatally and postnatally and refer as required
-Network and collaborate with program support and services
-Ensure that one to one contact is provided to infants, toddlers, and preschoolers and their families in their homes, or at the clinic to provide health education and link to community resources
-Maintain client records, reporting, according to contribution agreement and activities
-Participate in case conferences, staff meetings, workshops organized by other staff members and training sessions
-Other duties as required
Qualification and Education Requirements:
-Currently registered or eligible for registration with the Saskatchewan Registered Nurses Association in good standing
-One year experience working with First Nations and Community Health Nursing experience
-Must have a valid Saskatchewan Driver's License and vehicle at their disposal
Preferred Skills:
-The ideal candidate will be literate in computer skills, knowledge of Dakota traditions and culture would be an asset, proven experience in working with First Nations. Able to work independently with minimal supervision.
