Summary:
The DSW is responsible for planning and providing individualized support and/or to facilitate group activities for program participants. Following established person-centered plans, they support participants to maximize their potential and contribute to a positive quality of life. Programs are both community and center based.
Duties:
1. Develop effective working relationships with all staff and participants.
2. Support program participants to participate in programs such as supported work/volunteer work, fitness/leisure, recreation, community events et.
3. Offer support to encourage independence and assist to achieve personal goals.
4. Provide feedback for Personal Program Plans and Support Plans; monitors relevance and participation.
5. Documentation and reports.
6. Provide physical and/personal supports to participants as needed.
7. Provide transportation as needed.
8. Medication administration.
9. Cross-train for other duties.
Works alone with participants in the community and during transport.
Program locations subject to change.
Must have a valid Sask Driver's licence and regular access to a vehicle.
