Description

The Cognitive Disability Strategy (CDS) is an integrative team planning initiative that offers support to individuals with a cognitive disability. CDS Saskatoon Region is hiring a full-time Cognitive Disability Consultant based in Saskatoon.  Position start date is anticipated to be early to mid- August, 2019.
Primary duties but not inclusive to all duties:
Implementation of the Principles of Integrated Case Management and take on role of team lead for individuals and their integrated teams
Develop complex support plans with integrated teams for individuals who meet CDS criteria
Use the principles of person centered planning, strength based supports and person first language
Provide consultation to planning  to team leads and participate in, and support integrated  planning
Community Capacity Building within the Saskatoon CDS region and work collaboratively with the CDS provincial structure
Work to enhance the knowledge and skills of families and service providers in meeting the needs of individuals with cognitive disabilities who have behavioral and developmental challenges
Work as part of the Intake Committee to determine eligibility into the CDS
Coordinate and present community information sessions to promote increased awareness of CDS
Work with the local community service delivery mechanism to promote a team approach to providing services to individuals with cognitive disabilities
Have a working knowledge of behavior assessment, the development of positive behavior support plans and the implementation of plans
Requirements:
A university degree in the human services field, such as Social Work, Education, Psychology, or Nursing
Experience working in a team environment and with individuals with disabilities
Excellent communication and organizational skills
Criminal record check and a vulnerable sector check are required
Salary:
Starting at $64,500.00 annually
