DESCRIPTION
Under the supervision of the Senior Health Nurse, the Community Health Nurse will be responsible for providing care and community health nursing services to Sturgeon Lake First Nation specifically in the Communicable Disease Care Program and the Home Care Program.
QUALIFICATIONS
Degree in Nursing
Eligible for registration in Saskatchewan
Strong facilitation skills
Minimum 3-5 years related nursing experience required
Ability to work effectively within a holistic team approach
Willingness to attend community health training programs as required
Must adhere to SLFN Zero Tolerance Policy for Drugs and Alcohol
A current satisfactory Criminal Record (CPIC)
Valid drivers license and reliable vehicle
An ability to speak or understand cree is an asset
Strong commitment to health prevention, promotion and community development
Knowledge of Diabetes, Chronic Disease, Communicable Disease program and Home Care.
Able to work independently
A Sturgeon Lake Band Member would be an asset
DUTIES
Planning and conducting special education programs on specific subjects to improve the health status in the community.
Oversee and manage the Chronic Care, Communicable Disease, School Program
Assessing the health needs of community members and assisting their families or other programs/agencies to provide the necessary care for these individuals.
Recording appropriate information on Electronic Medical Record (EMR).
Assist in Primary Care Clinics and Immunization Program.
Carry out home care nursing duties.
Carrying out a comprehensive health program in the school. (ie: immunization, health education, etc)
Attend staff meetings other professional meetings, conference and trainings.
Other duties that may be assigned within your scope of practice.
