Facility: SWIFT CURRENT HOME CARE
Job Description: Under the direction of the Director Clinical Integration Continuing Care Rural SE and SW, the Patient Flow Manager will aide in arranging services to allow for efficient, well-organized discharges, ensuring the post operative needs of clients are in place for recovery. The incumbent will liaise and advocate for surgical patients from the Cypress Regional Hospital and larger tertiary centers in discharge planning to provide enhanced flow of clients and services within the region and between regions. The incumbent is also responsible for all long term care bed allocations within the former Cypress Health Region.


Required Qualifications

Bachelor Degree/Diploma in Nursing OR Degree in Allied Health Services or Administration/Commerce with experience in a wide variety of health care disciplines
Valid Class 5 Driver's license
Eligible for licensure with a regulated health care body.
Eligible for registration with SRNA
Experience
Minimum 3 years experience within the healthcare system
Understanding of the Healthcare system and related services within the Cypress Health Region
Excellent critical thinking skills
Knowledge, Skills and Abilities

Ability to work independently and as a member of a multi-disciplinary team
Excellent verbal and written communication skills
Excellent computer skills an asset
Excellent interpersonal and communication skills

Expected Start Date: August 6, 2019
Expected Up To Date: March 31, 2020
FTE: 1.00

Hours of Work: Shift Information: Days
37.50 hours per 1 week rotation
