Job Description: The Community Mental Health Nurse, as a member of an interdisciplinary treatment team, is accountable for the provision of screening, assessment, referral, case management and treatment of individuals with serious and persistent mental illness, and persons who may be at risk of developing disability due to mental illness. The Community Mental Health Nurse (CMHN) is accountable for the provision of education aimed at prevention and early intervention services to reduce incidence and disability due to mental illness.

The CMHN does not have direct supervisory responsibilities; he/she will work within a structure that emphasizes
interdisciplinary and community partnership in service delivery. This position reports to the Program manager of rehabilitation and community services.
The CMHN, as part of a Mental Health System, will be responsible and accountable for the provision of high quality psychiatric rehabilitation services through provision of intake, assessment, referrals, case management, treatment and service evaluation.

The CMHN will contribute to the development of effective community profile and relationships by utilizing excellent professional communication skills and forming quality interagency networks.

The CMHN is accountable to contribute to and maintain a team approach to service delivery through coordination, consultation and conferencing within an interdisciplinary, interagency and intersectoral service delivery framework.

The CMHN will provide support and consultation services to agencies throughout the service area.

The CMHN will arrange opportunities for exchange of information ideas and program needs to ensure consistent delivery of services within the service area.
