Under the supervision of the Program Manager, this individual is responsible for planning and implementing approved activities for Youth and Family Land Based Family Plans, will be the communication link between the referring organization, Family, Education, Wellness and other Agencies including on and off reserve programs, will provide written quarterly case conference reports on each client, arrange case conference meetings with the referring organization and the Families, will facilitate admissions and provide professional assessments to the case conference, will also focus on the holistic needs of each youth and family from Physical, Spiritual, Mental and Emotional perspective, on going life transition planning, implementation, direction and recommendation while maintaining focus on the individual life transition plans. QUALIFICATIONS: 1)BISW, BSW or a degree in a related discipline. 2)A combination of a Diploma in a related discipline plus a minimum of 5 years of experience will be considered. KNOWLEDGE SKILLS AND ABILITIES: Working in conjunction with the community programs and services, must possess the ability to assess and identify enhancement areas and the ability to refer and engage the appropriate resources. Excellent oral and written communication skills. Possess a valid drivers license and a reliable vehicle. Must be willing to upgrade skills when time and funding permits. Must be willing to work evenings and weekends. Must be able to sign an Oath of Confidentiality and a Non-disclosure Agreement. Excellent Computer Skills with Data Base knowledge. Knowledge of First Nations Beliefs, languages, customs and First Nations Group home standards, practices, policies and procedures. Knowledge of Indian Child and Family Services, Ministry of Social Services, Indigenous Services Canada, Saskatchewan Child Welfare Act, Child Welfare Policies & Procedures
