Job Description: In collaboration with Physicians and the Primary Care Team, the Nurse Practitioner will provide primary health care services to several communities. This includes: health promotion and protection, disease and injury prevention, provision of supportive, instructive, treatment and urgent services; collaboration with and referral to other health care professionals; conducting community needs assessments and participating in evaluative projects as needed.  The incumbent reports the PHC EMR Manager of the Saskatchewan Health Authority North East Rural Service Area.

Travel is required throughout North East Rural Network.
Chronic Disease Management and Home Care knowledge/experience an asset.

Work hours Monday - Friday (112 hours of work in 3 week period)
