Job Description: The Acute Care Coordinator assists the Manager of Acute Care Services in the establishment and maintenance of high standards of safe, patient care through the assessment of needs of staff of Acute Care. This will be accomplished though engagement of the Saskatchewan Health Authority Vision, Mission and Values. Facilitation coordination of resources and supports to meet employee needs and evaluation of those resources and supports.  The Acute Care Coordinator assists with the development, implementation and evaluation of nursing policies, procedures an expected behaviors based on evidence based best practice.  The Acute Care Coordinator assists nursing staff in implementation of best practice through identification and appropriate transfer of patient care to other services within and outside of the health region and/or discharge of patients with appropriate patient education and self management information.  The Acute Care Coordinator keeps current through maintaining licensure as a registered nurse and through providing direct patient care as determined in consultation with the Manager of Acute Care Services.

Education:
- Baccalaureate degree in Nursing
- Minimum of three to five years working in an acute care setting
- Current registration with Saskatchewan Registered Nurses Association
- Possess current CPR certification
- An equivalent of education and experience may be considered

KNOWLEDGE, SKILLS AND ABILITIES:
-    Organizational and leadership skills
-    Demonstrated ability to work in a collaborative, consultative manner
-    Excellent written and verbal communication skills
