Chip and Dale Homes Inc. is a Non-Profit Organization providing residential and day program services for adults with intellectual disabilities.

Applications are now being accepted for a Licensed Practical Nurse (LPN) who will be working at one of our group homes caring for four adults with complex medical needs. This is a part time job consisting of evening and night shifts on weekdays and weekends.

Duties Include:
Provide medical care within the full scope of practice as it is outlined for Licensed Practical Nurses.
Offer direct nursing care for medically fragile adults
Offer direct nursing care for complex medical, nursing and intervention therapies due to disabilities and/chronic diseases
Provide a holistic approach of health and healing along with nursing care for medically fragile adults
Assist in physical assessments
Administer medications, gastric tube feeds
Monitor and administer tracheotomy care including suctioning
Administer urostomy care, catheter care, diabetic care and specialized diets
Monitor and manage nursing care plans
Advocate on behalf of the individual's health care needs
Assisting Direct Care staff with household duties such as, cleaning, cooking, recreational activities, quality of lift activities, etc.

Qualifications:
LPN
Current license with SALPN
Minimum 2 years experience preferred
Must have a driver's license and vehicle.
