Facility: THE MEADOWS
Job Description: Reporting to the Manager(s) of Long Term Care Services, the Nurse Practitioner supports the mission, philosophy and objectives of The Saskatchewan Health Authority/The Meadows and is responsible for assessment planning, delivery and evaluation of health care services to the long term care residents and clients in the multipurpose beds of The Meadows. This is done in collaboration with the family physicians, other health care providers, community organizations and programs and the resident and their family. The Nurse Practitioner establishes and communicates diagnosis for common medical conditions through health assessment and synthesis of data, prescribing medications, ordering and interpreting of diagnostic tests, working within the full scope of practice of a Nurse Practitioner and Saskatchewan Health Authority policies and procedures.
Utilizes and synthesizes from multiple sources appropriate health assessment and differential diagnosis of client/patient status.Assess need for ordering and interpreting appropriate laboratory, radiology and ultrasound diagnostic tests. Evaluate the effectiveness of the resident/client care plan based on measurable resident/client outcomes with appropriate utilization of health services on an on-going basis.
Required Qualifications
Bachelor Degree in Nursing
Registered Nurse - Nurse Practitioner Program
Registered or eligible for registration with the Saskatchewan Registered Nurses Association (SRNA) as an RN (NP) or RN (GNP)
Valid Class 5 driver's license
Experience
Minimum of three (3) years experience working as a Registered Nurse, with experience as Nurse Practitioner in a long term care setting preferred;
