Job Description: THE HEPATITIS/HIV OUTREACH SOCIAL WORKER WILL WORK WITH AN INTERDISCIPLINARY TEAM TO PROVIDE CASE MANAGEMENT SERVICES AND HARM REDUCTION PRINCIPLES FOR CLIENTS OF THE TARGET GROUP, SIGNIFICANT OTHERS AND THOSE AT RISK.
Experience
WORK EXPERIENCE WITHIN HARM REDUCTION PROGRAMS AND CASE MANAGEMENT IN MENTAL HEALTH, IN COMMUNITY AND/OR CORRECTIONS. UNDERSTANDING OF ADDICTIONS, MENTAL HEALTH AND THE AFFECTS OF THE DETERMINANTS OF HEALTH. THOROUGH WORKING KNOWLEDGE OF HARM REDUCTION PRINCIPLES AND APPROACH

Applicant must be dedicated to excellent customer service and must demonstrate safety as a priority at all times.

112 hours of work in a 3 week rotation. Shifts(including actual hours, date of commencement and temporary end dates) are for informational purpose only and may be subject to change.
