Job Description: The Fall Prevention Coordinator is responsible for supporting the full integration of best practice fall prevention initiatives into everyday clinical practice throughout the continuum of care including acute care, home care, residential care and community.  The Fall Prevention Coordinator is accountable for the leadership, education, facilitation, quality improvement, evaluation and mentorship of the initiative and is also responsible for planning, developing, and implementing a seniors falls prevention plan for the Saskatoon Health Region.

The Fall Prevention Coordinator will work in collaboration with key stakeholders within the Saskatoon Health Region and within the Province.

Required Qualifications

Bachelor Degree in Occupational Therapy, Physical Therapy, Social Work, Nursing or Psychology or a related field
Registered or eligible for registration with the appropriate licensing body
Experience
5 - 7 years clinical experience working with older adults in multiple sectors - acute care, rehabilitation, long term care, and community with demonstrated leadership in program planning and development, implementation and evaluation, and recent experience in preventing falls in seniors and physical activity initiatives for older adults.

Hours of Work: Shift Information: Days
In a 3 week rotation: 14 shifts of 8.00 hours
