Job Duties:
-To provide accurate assessment of client needs and provide support services related to household management, life skills, medication management and counselling in accordance with individual treatment plans to individuals who are residing in the community
-work independently in the community as part of the Mental Health Home Care Transition Team to provide weekend support to assigned clients
-hours are 8 hours on Saturday and 8 hours on Sunday.
Education: preference for university degree in psychology or social work.
Experience: a minimum of 3 years related work experience.
