Under the direction of the Nurse in Charge, plans and carries out a Home and Community Care Nursing Program in conjunction with the William Charles Health Center (WCHC) Nursing Program objectives, guideline procedures and policies, and the needs of the community; functions as part of Home and Community care team; and perform other duties as delegated by Nurse in Charge.Ensures that the home & community program is based on a client referral assessment process. Assessing the needs of the chronically ill, person(s) with disabilities, psychiatric vulnerabilities, and the aged, and assisting the family or other agencies to provide necessary care to these individuals.Developing written care plans to meet the assessed short term and long terms needs of the homecare clients.Provide specialized nursing care for clients of the homecare program. Providing case management for home care clients. Promoting client independence by:
-Teaching and supervising self-care procedures to the client, the family and others.
-Providing general health care teaching and supervision of nutrition, maintenance of good health, safety and restoration of health.Evaluating client progress and updating of care plan based on ongoing nursing assessment. Promoting the maintenance of a safe and healthy environment in home and community.Documenting the appropriate information on the clients health records
QUALIFICATIONS:
Must have 5 years experience and preference will be given to those who have their RN/MN (Masters of Nursing).Registered with the Saskatchewan Registered Nurses Association as an RN.Maintains certification in CPR HCP annually.Obtains and maintains certification in Transfer of Dangerous Goods.High level of critical and logical thinking analysis, and/or reasoning to identify underlying principles, reasons, or facts.Experience working in a Cree community, knowledge of Cree culture and ability to communicate in Cree would be an asset.
