STR8 UP and the Community Intervention Model-
STR8 UP has entered into a contract with the Ministry of Corrections and Policing to provide gang-exit outreach services for the central and northern Saskatchewan and is looking for Outreach workers to provide support to adults and youth in correctional facilities and the community as they move through the justice system and transition out of the gang lifestyle to become responsible community members.
Responsibilities include:
Working with service agencies, criminal justice stakeholders and community;
Responsibility for contract requirements: client/staff ratio, risk-management, client programming, recordkeeping, financial and personnel reporting, and privacy;
Advocating in justice, corrections, social services sectors; and
With the member, conduct intake, ongoing case planning and self-assessments
Keep meticulous intake, case planning and attendance and referral records
Advocate in justice, corrections, social services sectors and for community supports
Provide one-to-one life skills and problem solving counselling
Conduct sharing circles in correctional facilities and meet with members in group settings for fellow-ship and programming
Accompany and support members attending important appointments
Assist in obtaining essentials to community living: Identification, financial assistance . Arrange access to spiritual and cultural practices to support healing
Help maintain family contact during incarceration and in the community
Facilitate recreational and cultural opportunities that include family
Arrange for addictions programming, housing, health, education/job skills and employment opportunities.
Participate in STR8UP presentations
Provide information about services and opportunities in the community
Identify partnerships for services in the community
Travel may be required to various communities throughout northern Saskatchewan
