Job Description: As a member of the interdisciplinary team, and under the general direction of the Manager of Rehabilitation and Community Services , the Occupational Therapist Degree is responsible for providing direct specialized care from a recovery service perspective that is inclusionary and flexible for individuals with moderate to severe and persistent mental illness, addiction and concurrent disorders.
Primary responsibility is to the provision of direct service in the community for individuals with moderate to severe level of persistent mental illness and /or addictions.  The team provides assessment, monitoring, team conferencing and delivery of timely and effective case management services to assigned clients.
    Promote client recovery, participation of family members and other significant individuals through application of recovery principles.
    Collaborates with other organizations involved in the delivery of mental health and addiction services. These organizations include but are not limited to, educational facilities, health agencies and professional organizations.
    Participates in clinical case conferences, ensuring client progress and documentation.
    Assesses complex addictions and mental health situations and plans and conducts appropriate programs, group therapy, mediation, and referral to and collaborative efforts with other agencies in accordance with evidence based practice.
    Develops recovery plans with clients.
    Participate in team/program activities to enhance and optimize the delivery of Recovery oriented services.
    Maintain endorsed documentation, compiles and submits statistical information and reports as required.
    Conducts counseling and group therapy sessions meeting clients needs and in the problem solving process.
