Job Description: Community Team Manager is a delivery model that utilizes integrated, interdisciplinary teams of health professionals and community members to provide high quality service as close to home as possible, at the most appropriate time, by the most appropriate person.  The Community Team Manager reports to the Director of Primary Health Care and is accountable for the PHC team members in Spiritwood and the surrounding area. This position provides leadership in team building, clinical excellence, and interdisciplinary team practice within their PHC sites, as well as ensuring the philosophy, objectives, and policies of the Saskatchewan Health Authority Home Care program are in alignment with the SK Ministry of Health.
Required Qualifications

Bachelor Degree in Nursing
Registered or eligible for registration with the Saskatchewan Registered Nurses Association (SRNA)
Experience
- 3-5 years experience in community nursing with demonstrated leadership in nursing practice or community care coordination preferred
- Experience working with a PHC team is an asset
- Leadership and/or supervisory experience are required
- Relevant advanced education/experience related to team facilitation, community development, health systems, policy development, evaluation and research
- Proficient with computer software (Work, Power Point, Excel, Access, Outlook)
- Excellent written and verbal communication skills for individual, small group, and large group interaction both within the region and the community
- Demonstrated commitment to own professional growth and development
