Job Description: Under the direction of the Manger of Rehabilitation and Community Services, and as part of an interdisciplinary team, the Social Worker BSW provides a range of screening, triage, assessment, treatment and case coordination services to primarily adult community mental health and addiction population.

-Utilizes a team approach to provide screening and/or assessments; prioritizing referrals; provides suicide risk assessments; crisis intervention; family support services; case management services; clinical assessment and effective interventions to persons suffering from mental health and addictions issues.

-Provides education and support to others in the community regarding mental health and addiction client needs and mental health and addiction issues
-Application of the Mental Health Services Act and Health Information Protection Act
-Provides coverage to client's receiving services
-Maintains client records as per SHA documentation standards
-Participates in clinical audits and attends meetings as required
-Consults with program area manager, Sr. Clinicians, Discipline Heads, and other colleagues as appropriate regarding difficult clinical situations, legal and ethical issues.
-Works collaboratively with both internal and external service providers of the SHA in the provision of services to individuals with mental health and addictions issues
-liaise and advocate with various community agencies to ensure that clients with mental health and addiction issues can access and utilize community services appropriately
-Assessment of community resources, identifying needs and gaps in services
-Participates in assigned community development projects as required
-Responsible for conducting self in a safe and responsible manner as outlined in the Occupational Health and Safety Act.
