POSITION SUMMARY
As part of a dynamic team in the Crisis Shelter and Residence, the Mental Health Case worker provides critical daily supports and services to women residing in the mental health and addictions beds. The goal is to meet women where they are in their treatment plans and support through their healing journey and transition.
MAJOR RESPONSIBILITIES: Working as part of a collaborative team:
Provide respectful and compassionate counseling and support services to women accessing services
Conduct intakes and assessment of client assets and needs
Case coordination and client advocacy
Provide daily care and support
Intensive case management
Conduct intakes and discharge plans
De-escalate, manage crisis and volatile situations
Maintain ethical record keeping, documentation and timely reporting
Maintain a safe and comfortable environment for all residents
Liaise with other professionals working with the residents treatment plan and provide referrals as necessary
Work collaboratively as a team and with other program areas in CS&R
Provide transportation as necessary
