Facility: VICTORIA HOSPITAL
Job Description: REPORTING TO THE DIRECTOR OF THE PATIENT CARE COORDINATOR UNIT, THE ACUTE CARE ACCESS PROGRAM (ACAP) ASSESSOR/COORDINATOR, IS RESPONSIBLE FOR PROVIDING ACCESS FOR CLIENTS AT RISK OF ADMISSION TO HOSPITAL. THE ASSESSOR/COORDINATOR WORKS WITHIN A TEAM TO DETERMINE APPROPRIATENESS OF ADMISSION AND BED UTILIZATION AND HELPS TO INTIATE CARE PLANS.  THE ASSESSOR/COORDINATOR ASSIGNS CLIENT CARE TO A PATIENT CARE COORDINATOR AND ACTS AS A RESOURCE/CONSULTANT TO THE UNIT.
Required Qualifications

Valid Class 5 driver's license
Registered or eligible for registration with the appropriate licensing body
Bachelor Degree in Social Work, Nursing, or Psychiatric Nursing
Experience
2 YEARS EXPERIENCE IS REQUIRED
Knowledge, Skills and Abilities

Change management skills
Ability to work independently and as a member of a multi-disciplinary team
Ability to communicate with and maintain an effective working relationship with clients from diverse social, economic and cultural backgrounds
Ability to competently use computers and software applications as required by the position
Advanced leadership skills
Assessment skills
Case management skills

282.72 hours of work in a 12 week rotation.

The successful applicant(s) must provide a satisfactory valid Criminal Record Check (which may include a Vulnerable Sector query) before an Offer of Employment can be made.
