Facility: Nurses Alumni Wing
Job Description: The Community Mental Health Nurse Transition Team is a member of Mental Health and Addiction Services.  The nurse is an integral part of a multi-disciplinary team providing comprehensive assessment, including suicide risk assessment, crisis interventions and short term intensive case management to those clients seen in ER or Dube Center for Mental Health and referred for services on discharge from those facilities.  These services are provided within an individual context.  Responsibilities include intensive case management, medication management, crisis intervention, counselling, goal planning, consultation to community agencies, group/community education, student mentoring, documentation, client/family and public education, advocacy, and mental health promotion.
Required Qualifications

Bachelor Degree/Diploma in Nursing or Bachelor Degree/Diploma in Psychiatric Nursing
PART Basic Level
Registered or eligible for registration with the Registered Psychiatric Nurses Association of Saskatchewan (RPNAS) or Saskatchewan Registered Nurses Association (SRNA)
Workplace Assessment and Violence Education Certificate
Basic Life Support (BLS) - Level C Health Care Provider (HCP) - Heart & Stroke
Current CPR Certification
Knowledge, Skills and Abilities
