Job Description: This social worker collaborates with the interdisciplinary team on the nursing unit, in community nursing and with the Intake program.

Working with staff on the unit the incumbent will be responsible to provide discharge planning for select clients by coordinating & participating in family meetings, arranging for services through community partners & by assisting the client with housing and finances pre-discharge and 2 weeks post discharge.

Working with the Community Mental Health Nursing Department the incumbent will be responsible in a consultant role to identify and assist with available community resources in the community of North Battleford. In this capacity the incumbent will also be the key contact for approximately 15 approved home operators in North Battleford who will consult with her / him in regards to legislative requirements and client placement.

Working with the intake team 1 day per week the incumbent will be responsible for assessing / triaging mental health clients who are presenting for service on either a walk in basis, as a referral from a family doctor or as a referral from another department / agency.


Required Qualifications

Bachelor Degree in Social Work
Valid Class 5 driver's license
Registered or eligible for registration with the Saskatchewan Association of Social Workers (SASW)
Knowledge, Skills and Abilities

Ability to work independently and as a member of a multi-disciplinary team
Ability to adapt to changing conditions
