The Jordans Principle Service Coordinator:
* Will provide enhanced service coordination to the children and their families living on BRT6HC communities to ensure the children and their families have access to the health and social services they need, when they need them and provide advocacy support to ensure that they do not experience denials, delays or disruptions of services due to jurisdictional disputes.
* Will work collaboratively with BRT6HC communities, Battlefords Early Childhood Intervention Program (BECIP), partnering organizations and existing community-based services to work towards establishing multi-disciplinary hubs for families when the services they require involves a number of professionals
Knowledge, Skills and Abilities
* Early Childhood Education certificate/ or equivalent training
* Demonstrate an understanding of the health and social issues encountered by First Nation people
* Knowledge and respect for the culture, traditions and customs of the local First Nation communities and a willingness to learn about and accept First Nations cultures
* An understanding of Federal, Provincial, and First Nations Health Care Systems plus knowledge, skill and ability to learn about health agencies and available programs
* Ability to work independently and within a team environment.
* Ability to take initiative and meet deadlines.
* Excellent problem solving and organizational skills.
* Experience in case conferencing management
* Excellent writing, communication and presentation skills.
* Ability to establish and maintain cooperative working relationships with staff, clients and community service agencies.
* Ability to work with confidential and highly sensitive and personal information
* Experience with Microsoft Word, Excel and Power Point, and electronic management of client files
* Valid class 5 drivers license
* Ability to speak and understand Cree
* Must provide a successful criminal record check with vulnerable sector
