The Community Health Nurse will collaborate with community staff in the delivery of the following programs in a way that is both culturally safe and responsive to the identified needs of the First Nation.
MANDATORY COMMUNITY HEALTH PROGRAMS:
The following are required services to be delivered to all ages throughout the lifespan.
a) Communicable Disease Control Program including Tuberculosis Control
b) Immunization Program
ESSENTIAL COMMUNITY HEALTH PROGRGAMS
a) Maternal Health
b) Infant Program
c) Preschool Program
d) School Program
e) Administrative Functions
f) Professional Development
COMMUNITY IDENTIFIED HEALTH PROGRAMS
Examples:
a) Adult Health
b) Mental Wellness
c) Environmental Health
d) Diabetes Awareness and Prevention
e) Harm Reduction
f) Emergency Preparedness Planning
Administrative Functions
The Community Health Nurse is expected to maintain all programming and policies in an organized and comprehensive manner including (but not limited to):
o Charting as per agency guidelines (Charting Manual)
o Keeping district book up to date and accurate.
o Participating in chart audits
o Completing other administrative reports
o Ensuring maintenance of nursing equipment
Community Identified Health Programs
It is important for the Community Health Nurse to collaborate with other community based staff and work to meet identified needs where possible in areas such as:
o Supporting activities of other programs in the Health Centre
o Attending health committee meetings to provide information about the nursing program
o Liaison with other programs as they relate to nursing (ie. education, AHS, Day Care).
o Responding to requests for information/education
o Participating in community organized events, where appropriate and where possible, in areas such as adult health, mental wellness, environmental health, diabetes awareness, harm reduction and emergency preparedness
