Job Description: Under the direction of the Manager of the Pediatric Inpatient Department, the Nurse Coordinator is responsible for the coordination of care and ensuring continuity of care for the pediatric patient with end stage renal failure requiring renal replacement therapy and/or transplant. The incumbent will be responsible for assisting with the development and ongoing coordination of the provincial hemodialysis and kidney transplant program. The Nurse Coordinator must work cross functionally within the portfolio and across the organization to support operations and services locally and provincially. Assisting the patient and family to coordinate and access timely, appropriate care through the health care system efficiently. The incumbent will develop and foster relationships with other health care providers including community partners and agencies to gain familiarity with and mobilize resources for the patient and family. Additional responsibilities include quality improvement and research initiatives.
Required Qualifications

Registered or eligible for registration with the Saskatchewan Registered Nurses Association (SRNA)
Care of the Patient with Kidney Disease
Baccalaureate Degree in Nursing
Experience
Preference for a minimum of three (3) years of recent experience of caring for children with chronic kidney disease or kidney failure, including experience in renal replacement treatment.
Experience related to leadership, case management, care coordination, patient education, program development and evaluation.
Experience with a transplant program or transplant care is an asset.
Knowledge, Skills and Abilities

Proficiency with a variety of computer systems and software applications
