The Client Navigator will be based out of STC Health Centre and supervised by the Health Centre Manager. The Client Navigator will work with the health team, and associated external medical and community services to assist clients to address and engage services that will improve their health and quality of life. The Client Navigator will perform assessments and utilize a client centered case management approach to address the clients short-term and long-term education, referral, and support needs.
