Job Description: Reporting to the Transition Lead, Pioneers Housing-LTC, the Resident Care Coordinator is responsible for ensuring the highest standard of care and service provision to residents.  As an experienced practitioner, the resident care coordinator acts in a leadership role to provide specialized clinical expertise, coordination of resident care, and support and teaching to the resident care team (including staff, residents and residents, families) within a collaborative environment.  The routine procedures are generally predetermined but critical thinking and independent judgment is an integral part of the position.


Required Qualifications

Registered or eligible for registration with the Saskatchewan Registered Nurses Association (SRNA) or Registered Psychiatric Nurses Association of Saskatchewan (RPNAS)
Baccalaureate Degree in Nursing preferred
Knowledge, Skills and Abilities

Project management skills
Supervisory skills
Ability to apply research to nursing practice
Demonstrated ability to work within a team environment
Demonstration of extreme attention to detail
Demonstrates effective organizational skills for multiple projects and deadlines
Makes judgements on the basis of critical thinking and problem solving criteria
Demonstrated initiative, self-direction, organizational and leadership abilities
Demonstrated ability to establish and maintain effective interpersonal/consultative relationships and communication patterns with all contacts and with other health professionals of the interdisciplinary team
Ability to communicate effectively both orally and in writing with clients of varying ages, socio-economic status, and targeted populatio
