The RN will coordinate homecare staffing services to meet the needs of the homecare clients needs based on client care plans, while encouraging client independence, family support and community support. The HCC RN will also plan, develop, and evaluate workplans of the Home and Community Care service, ensuring these services are consistent with the philosophy, objectives and policies of the Saskatchewan Home and Community Care Program and George Gordon First Nation Community Health Plan.
Duties/Responsibilities:
1. Program Service Delivery
Perform assessments/reassessments using the Saskatchewan Home and Community Care assessment tools
Develop care plans based on assessments.
Provide Case Management for all homecare clients
Perform nursing skills required to meet clients nursing needs
Maintain accurate client files
Prepare staff assignments to the Certified Home Health Aides and Licensed Practical Nurse(s)
Work collaboratively with the GGFN Homecare team, Aboriginal Diabetes Educator, Community Health Team, and the Wellness team.
Act as the GGFN Hospital liaison for clients that are admitted to hospital or identified by hospital SWADD team as needing homecare services on discharge from hospital
2. Supervision
Supervise the GGFN Home and Community Care team: Certified Home Health Aides and the Licensed Practical Nurse(s)
Ensure HCC staff are documenting client care accordingly in their chosen charting system
Complete monthly eSDRT reports and health committee reports; quarterly assisted living reports; annual CBRT reports, and PIP and Homecare Summary Reports.
3. Program Development
Participate in planning, development and evaluation of the Home and Community Care Program
