Job Description: The Client Care Coordinator is accountable for assisting the Home Care Manager to ensure efficient and effective coordination and delivery of home care services in the assigned area. This position ensures a high quality of client care in keeping with the philosophy, objective, policies, and procedures established by the SHA. The Client Care Coordinator through a collaborative case management process is accountable to ensure that the client has access to appropriate, adequate, high quality, coordinated and integrated service that is continuous throughout the healthcare agencies (system.) The Client Care Coordinator must utilize a collaborative client centered process that respects the clients dignity, the clients right to make informed decisions and the role of family, caregivers and community resources in planning care and caring for clients. The Client Care Coordinator promotes quality care and cost effective outcomes while addressing the health and well being of the client based on the clients unmet needs by assessing, planning, coordinating, implementing, monitoring and evaluating services received by the client. This position has staff supervisory responsibilities.
Required Qualifications

Valid Class 5 driver's license
Registered or eligible for registration with the appropriate licensing body
Bachelor Degree in Nursing or Bachelor Degree in Social Work or Bachelor Degree in Physiotherapy or Bachelor Degree in Occupational Therapy
Experience
One year related professional experience working with geriatric clients and/or adults living with a disability in community health care preferred.
