Job Description: The Patient Care Supervisor, reporting to the Manager of Primary Care, organizes and supervises all the administrative activities that facilitate the smooth and effective operation of the Saskatchewan Health Authority (SHA) Creighton Home Care Office.  The incumbent is responsible for scheduling, training and supervision of administration and clinical staff.

QUALIFICATIONS
    Current SRNA registration
    Degree or Diploma from an approved School of Nursing
    Willingness to maintain or obtain required competencies in practice
    Demonstrated understanding of case management as part of the service delivery approach
    Self motivated and self directed
    Three years related experience in Home Care preferred
    Computer literacy in email, word documents and web search.
    Working knowledge of the Procura program and RAI-HC assessment tool preferred.
    Ability to enter data reliably and efficiently through keyboarding
    Valid Saskatchewan Drivers License and access to a reliable vehicle
    Ability to travel extensively and drive on rural roads

SKILLS
    Strong leadership, management and organizational skills as well as an understanding of community-based programs;
    Ability to integrate and harmonize diversified functions between staff;
    Ability to multi-task;
    Strong communication skills, a good decision maker;
    An understanding of the Patient and Family First philosophy
    Sound knowledge of the nursing process
    Interacts with clients, families and staff in a tactful, meaningful way
    Ability to handle several complex issues concurrently, while maintaining attention to detail
    Ability to work independently and as a leader of a multidisciplinary team
    Must be a client advocate
    Must be culturally sensitive when dealing with multi-cultural clients and co-workers
