Facility: PUBLIC HEALTH - SHR
Job Description: The Assessor Coordinator will be accountable for providing access and case management services for clients/patients within SHA. The Coordinator provides information and facilitates access to a variety of resources, internal and external to SHA. The Coordinator acts as a liaison and advocate for clients/patients ensuring that those clients with the greatest need have access to service in a timely manner.
Required Qualifications
A degree in nursing, social work, physical therapy or occupational therapy.
Experience
Two years recent client assessment experience, preferably in a health care or social service setting.
Knowledge, Skills and Abilities
Communication skills
Ability to work independently to plan and organize work load demands
Ability to work with a diverse group of clients
Knowledge of client case management and process facilitation
A driver's license and capacity to travel and to transport test kits and other materials needed to complete assessment work in community.
Ability to problem solve and make decisions in a timely manner
Demonstrated skill in advocacy, negotiation and conflict resolution
Word processing skills necessary
Ability to interact with or work in a team setting
Demonstrated knowledge of community resources, internal and external to SHA
Knowledge of/or willingness to become familiar with medical terminology
Expected Start Date: January 27, 2021
Expected Up To Date: January 13, 2022
FTE: 1.00
Hours of Work: Shift Information: Days, Evenings
224.00 hours per 6 week rotation
