Job Description: The Senior Assessor Coordinator is a supervisory, team leader position at the Moose Jaw and Area Access Centre reporting to the Primary Health Care and Clinical Integration Management teams. As a senior practitioner, the incumbent acts as a resource for other professionals in the SHA as well as across other provincial ministries. The position will provide clinical supervision for Assessor Coordinators in the functions of screening, intake, assessment, care planning and case management practice.  The Senior Coordinator is responsible for taking a leadership role with scheduling and providing direction to service providers in Primary Health Care.  This position also provides linkages to other programs and services as required by clients and programs. The position requires the use of high order knowledge, clinical expertise and strong interpersonal skills to coach, mentor, orientate and provide education to the team members. Drawing on standards of practice for case management; professional codes of ethics and standards of practice in health care; program policy and procedures; and, relevant legislation, the incumbent will ensure that the team provides high quality client service and professional standards of practice are maintained.

The assignment of time may be:
- assigned case management responsibilities to a patient/clinical area and/or
- supervising complex clinical cases, supporting, coaching, mentoring, and orientating members to Primary Health Care and/or
- involved in development, implementation and evaluation of projects related to case management practice

Required Qualifications

Recognized degree/diploma in nursing or social work degree
Must have a current practicing registration with their professional association
