Direct Support Professional-Community Day Supports
Prince Albert
Full-Time
In conjunction with the people supported and their natural support network (families, advocates), assist
to:
- Assess individual needs
- Plan and implement programs to meet needs (using Christian Horizons systems)
- Ensure accurate collection of data and documentation to maintain records and to provide
information to professionals as necessary
- Access community supports and other care professions to meet the persons supported needs
(behaviour, physical, emotional, spiritual, personal development, communication)
- Advocate for access to community activities/programs/friendships
- Develop, advocate and monitor opportunities for access to employment or volunteerism
- Offer educational opportunities to people supported
- Participate in program evaluations
- Promote and market products as required
Requirements:
3-6 Months Experience.
Developmental Services Worker, Child and Youth Worker, or post-secondary education in a related field, is preferred;
Satisfactory Vulnerable Sector Police Check;
Valid class 5 Drivers License and access to reliable transportation.
