Phoenix Residential Society, a community based health care organization, has an opening for a full time position as Intensive Case Manager in our Housing and other Management and Engagement Services (HOMES) for a full-time position.

Working under the overall direction of the Manager of Homelessness Initiatives, the Intensive Case Manager position is an integral part of a team-based approach in the provision of support services, referrals, advocacy, and system navigation to those individuals who are chronically homeless.  The position entails intensive case management for individuals residing in a 24-hour staffed facility.
