Job Description: The Mental Health and Addiction Service (MHAS) Community Outreach Nurse Practitioner RN (NP) is an advanced practice nurse with knowledge and skills gained through additional education and experience. The Nurse Practitioner blends an in-depth knowledge of nursing with the legal authority to order diagnostic tests, prescribe pharmaceuticals and perform specific procedures in the management and treatment of common acute illnesses and stable chronic conditions. The RN (NP) practices within a framework of evidence-based primary care and mental and health and addictions care. The RN (NP) promotes holistic, promotion and prevention care with an understanding of the determinants of health. The practice involves building partnerships with individuals, families and communities. This includes community needs assessment, supporting inter-professional collaboration and promoting continuous improvement. The Nurse Practitioner will work closely with the Manager of Mental Health and Addiction Services - Adult Outreach and Targeted Prevention Services to supervise and evaluate nursing staff, provide leadership and function as a role model and mentor to assist staff with professional development and achievement of Mental Health and Addiction Services program goals. The primary functions of this position are researching best practices; educating community based mental health and addictions (RN's) nurses on best practices. The RN (NP) is responsible for providing clinical supervision to community nurses and to successfully manage, support and sustain change. The RN (NP) will have the responsibilities of clinical team building and clinical supervision within a collaborative community nursing model of care.
