Description

Working closely with the entire Support Services Department, Case Managers provide both in-house and outreach-based support to people in our community with a variety of needs. Case Managers assist individuals in accessing resources in the community such as housing, healthcare, and justice support.

-Act as an advocate for people who access services (PWAS)
-Conduct yourself in a manner that maintains and promotes a positive image for Prairie Harm Reduction, and is in keeping with the policies, mission, goals, and philosophies of the organization
-Work collaboratively with other programs in the building to ensure that programs are operating
-Offer naloxone training to PWAS (train and distribute kits as needed)
-Provide ongoing support, information, advocacy, referrals and system navigation to PWAS
-Provide transportation for PWAS as part of their case plan
-Maintain high knowledge of income assistance policies
-Help PWAS navigate various systems (ie. justice, healthcare, human rights)
-Maintain a high knowledge of HIV, Hepatitis C, harm reduction, and trauma-informed care
-Maintain adequate knowledge and skills to ensure PWAS receives a high standard of services
-Maintain detailed statistics in the communications log for case management and reporting including collating monthly statistics
-Help with Christmas Hampers Program
-Help plan and implement PHRs various fundraisers
-Work collaboratively with community partners
-Attend community events as directed
-Attend training and professional development as instructed by the Director of Support Services
-Other duties as assigned
